How To Enroll

New Student Registration Dates:
August 13-16:
Hours: 8:00am to 12:30pm, 1:30pm to 3:30pm
Lunch Break: 12:30pm to 1:30pm
 
To register with the district, parents or guardians must register children at the appropriate campus(es). The following paperwork is needed for registration:
 
Two (2) Proofs of Residency: Current water or electric bill and a lease agreement or mortgage documentation showing proof of current residency
 
Birth Certificate
 
Current Immunization Records. Immunizations for each child must be up-to-date before entering school. Children MUST be immunized for DTP, Polio, Measles, Mumps, Rubella, HIB, Hep-B, and Varicella (Chickenpox).
 
Custody papers needed if you are not the student's parent or legal guardian.
 
Transcript/withdrawal papers from previous school.
 
Driver's license of person enrolling student.
 
 

Transfer Request
Parents who wish to transfer their child from one Midlothian ISD campus to another MISD campus must fill out this form and return it to the principal at their assigned campus. If necessary, additional written documentation to further communicate the circumstances of your situation will be accepted. Click here to download the form.
